How IEHP Works

IEHP wants you to have the best health care possible. If you are unhappy with your care for any reason, we encourage you to speak with your Doctor or Provider. Very often, problems can be solved through simple and honest communication.

Or if you prefer, call IEHP Member Services 1-800-440-IEHP (4347)/TTY 1-800-718-4347. All our Member Services Representatives speak English and Spanish and will work with you to clear up any issues you may have.

If you consider this issue to be urgent, please call IEHP Member Services, or you can call directly to the Department of Managed Health Care at 1-888-HMO-2219/TDD line at 1-877-688-9891.

If you are still unhappy, you can file a complaint.

If you feel that your problem has not been resolved, you have the right to file a complaint, also known as a “grievance.”

An IEHP Member Services Representative can explain the Grievance Process to you and assist you in filing your grievance, or you can use a Member Complaint Form. You can get a form in your Doctor’s office, we can mail you one, or you can click here to file your Grievance online.

You can also print out the Grievance Form in PDF format (see choices below)or (Text Only) and send it in to us by fax at (909) 890-5748, by mail at P.O. Box 19026, San Bernardino, CA 92423-9026 or in person at 303 E. Vanderbilt Way, San Bernardino, CA 92408. Please select the Grievance form appropriate for your use:
