As an IEHP Member, you can take advantage of IEHP’s Health Management Program
at no cost.

Whether you have asthma, diabetes or if you are a
mom-to-be, you can get personalized help so you will feel better, have more energy and know that you are in control of your health.

Here's what you'll get:

If you have asthma or diabetes

· Develop a personal relationship with a care coordinator specializing in asthma or diabetes.
· Create a customized management plan that fits your needs and keeps you out of the emergency room.
· Learn how your medications work and how to take them.
· Understand the numbers: whether it’s your blood sugar levels or your peak flow meter, you’ll know why these numbers are so important to your health.
· Learn how to keep your numbers in a healthy range, and what to do when they’re not—like calling your Doctor or the After-Hours Nurse Advice Line.
· Learn how diet and physical activity can improve your daily life.
· Your care coordinator will help you work with your Doctor and will stay in touch with you to make sure you’re doing okay.

For moms-to-be

· Develop a personal relationship with a care coordinator specializing in helping Members who are pregnant.
· Learn why prenatal care will help you have the healthiest baby possible.
· Learn what simple tests you need to make sure your baby is growing well.
· Learn how diet and physical activity are good for you and your baby.
· Your care coordinator will help you work with your Doctor and will stay in touch with you to make sure you’re doing okay.
